Privacy Policy

I understand that Rehab It Physical Therapy, P.C. is required by law, professional ethics, and guidelines to maintain the privacy of my protected health information (PHI) and may use or disclose my PHI for the purposes of carrying out treatment, obtaining payment, evaluating the quality of services provided and any other administrative operations related to such treatment or payment.

You will be asked to provide written consent to Rehab It Physical Therapy, P.C. to share relevant health information and/or copies of any medical records to my physicians as needed to coordinate care. At times, records may be requested from my physicians in order to provide the most effective care, such as surgery notes or progress notes.

I may revoke permission for any individual physician or all physicians at any time by providing written revocation to Rehab It Physical Therapy, P.C. In order to facilitate my care, Rehab It Physical Therapy, P.C. may also share relevant health information and discuss my care with the below listed providers (examples: acupuncturist, doula, massage therapist, yoga instructor, personal trainer) upon your written approval.

I will give permission on a case-by-case basis to Rehab It Physical Therapy, P.C. to any new providers. I may revoke this permission for any individual provider at any time by sending written revocation to Rehab It Physical Therapy, P.C.

A Notice of Privacy Practice is available upon request for your records. The SMS opt in consent, personal information, insurance information and the phone numbers that we collect will not be shared with third parties under any circumstances.
