Rehab It Wellness Agreement & Policies

Our client agreement and policies are designed to provide clear expectations around your wellness journey, including scheduling, communication and general guidelines to ensure a smooth and consistent experience. They also outline important information regarding consent, privacy, and participation so you can feel informed and confident moving forward with your sessions.

Agreement & Policies

The Policies, Liability Waiver and Release (“Agreement”) is entered into between the undersigned participant (“Participant”) and Rehab It Wellness, LLC, its owners, employees, contractors, trainers, and affiliates (collectively referred to as “Rehab It”).

Participant understands that participation in fitness training, assisted stretching, mobility work, recovery sessions, strength training, conditioning, and related wellness services (collectively referred to as “Training Activities”) involves inherent risks.

These services may include, but are not limited to: strength and conditioning exercises, plyometrics, sports specific training, mobility and flexibility training, assisted stretching or passive stretching performed by a trainer, recovery techniques, balance and stability work, movement training and corrective exercise. Participant understands that these activities may involve physical contact and manipulation of limbs or joints and may carry risks including, but not limited to: muscle strains or tears, ligament sprains, joint irritation, temporary soreness, exacerbation of existing injuries.Participant voluntarily chooses to participate with full knowledge and acceptance of these risks.

Participant understands that the training services provided are not physical therapy or medical treatment and are intended solely for general fitness, strength, and wellness purposes. These services may include assisted stretching and recovery techniques but do not constitute medical treatment, manual therapy, or physical therapy services.

Participation in Training Activities does not create a physical therapist–patient relationship unless the participant is separately evaluated and treated as a patient by a licensed physical therapist through Rehab It Physical Therapy. Participants are encouraged to consult with their physician prior to beginning any exercise program.

Participant certifies that they:
1. Are physically able to participate in exercise activities
2. Have disclosed any known medical conditions, injuries, or limitations that may affect participation
3. Will immediately notify the trainer of any pain, discomfort, dizziness, or unusual symptoms during training
4. Participant agrees that they assume full responsibility for their health and physical condition.

Participant knowingly and voluntarily assumes all risks associated with participation in Training Activities, including those that may arise from negligence. To the fullest extent permitted by law, Participant releases and holds harmless Rehab It Wellness, LLC, its owners, physical therapists, trainers, employees, contractors, and affiliates from any and all claims, liabilities, damages, injuries, or losses arising from participation in Training Activities.

This release applies to any injury, illness, or damage sustained during or as a result of training sessions. Participant agrees to indemnify and hold harmless Rehab It and its trainers from any claims, damages, or expenses arising from their participation in Training Activities.

 

IN HOME SAFETY & CONDUCT: Participant acknowledges that in-home sessions take place in a space selected and controlled by the Participant and agrees to provide a safe, clean, and appropriate environment free of hazards. Rehab It is not responsible for any injuries or damages resulting from unsafe or uncontrolled conditions within the home.

 

Participants are expected to maintain a professional and respectful environment at all times. Any inappropriate behavior, including discrimination, harassment or sexual misconduct toward staff, will result in immediate termination of the session and may lead to discontinuation of services without refund. Rehab It Wellness reserves the right to refuse or discontinue services at its discretion if conditions or conduct are deemed unsafe or inappropriate.

EMERGENCY MEDICAL TREATMENT: Participant authorizes Rehab It staff or trainers to obtain emergency medical treatment if deemed necessary during training sessions. Participant agrees to be responsible for any costs associated with such treatment.

CONSENT FOR TREATMENT OF MINOR: If Participant is under 18 years old and as the legal parent/guardian I am consenting to patient participating in Training Activities in the event a parent/guardian cannot be present for their session.

BILLING POLICY: I want the financial side of your experience to be straightforward and transparent. Services are not billable through health insurance. Payment is due at the time of service. All packages expire within 1 year of purchase. Monthly memberships expire at the end of each calander month.

CANCELLATION POLICY: Your session time is reserved for you alone. Please do your best to keep your scheduled time in order to reach your goals. We understand that unforeseen events happen in life, but please inform us of your need to cancel or reschedule as soon as possible. There may be a $50 fee for sessions canceled within 24 hours. Sessions that are canceled/rescheduled within 4 hours or missed/no show appointments will be charged the full amount. We do acknowledge extenuating circumstances and may take them in consideration when charging the fee. If you are late, I will do my best to accommodate you for the full time of your session, but I cannot guarantee the full appointment time.

CONSENT FOR DIGITAL CONTENT: Photographs and videos may be taken during visits for the purpose of client education and training. To allow for comparison over time, these files will be stored on your Rehab It's personal, secure devices. I consent to the use of this digital content in a professional manner. I may elect to opt out of this at any time and will notify Rehab It in writing.

SOCIAL MEDIA: Photographs and videos may be taken during visits for the use of marketing and/or promotional purposes and may be published on our website and social media platforms (Instagram/Facebook). Confidential client information will not be contained in this content. I consent to the use of photographs and videos of me in this manner. I may elect to opt out of this at any time and will notify Rehab It in writing if I wish to do so. I understand that the revocation will only apply on a go-forward basis and will not apply to any content that has already been published based on this authorization.

CONSENT FOR ELECTRONIC COMMUNICATION: I hereby give my consent for the use of email, voice and/or text messaging to communicate about my care with Rehab It, including pending sessions and follow up between visits. I understand that such communications may include personal healthcare information and that such transmissions are not encrypted. Such communications are limited to me and parties with whom I give written permission to communicate. At no time will such contact information be shared or publicized. I understand that I retain the right to revoke this consent by notifying Rehab It in writing at any time and subject to written confirmation of such notice.

ACKNOWLEDGMENT OF UNDERSTANDING: Participant confirms that they have read this waiver in its entirety, understand its contents, and voluntarily agree to its terms. Participant understands that by signing this agreement they are waiving certain legal rights.
